• KY or IN Immunization Certificate To meet Department of Health standards, each student is required to have a current immunization record (available from your physician) on file with the school. OR KY or IN Immunization Waiver: If your family has  an objection to providing these records, you must submit this Immunization Waiver. Students will not be able to start school without either records or a notarized waiver.
• Physical Exam for Athletics: All students participating in an extra-curricular athletic activity at HLS need to have a current (within one year) physical examination form on file in the school office. This includes sanctioned sports (basketball, CC, volleyball, etc.) as well as intramural sports. You may use this form for students who participate in Athletics at HLS: 2022 Physical Exam Form

Students participating in Varsity athletics may be required to provide a copy of their birth certificate.
